HOW TO USE (5 STEPS)
1. Fill in the client name, document title and date at the top.
2. Mark the pages that need a signature.
3. Send this cover sheet on top of the document.
4. Ask the client to sign each marked spot and date it.
5. Ask them to return the signed copy by the method shown on the sheet.

REQUIREMENTS
Any PDF reader and a printer, or a PDF editor for digital use.
